Consultation forms

Hi everyone.
Wasn't too sure where to post this but its a general question so thought this was the best place

How long are we supposed to keep consultation forms for? and also if a client has a consultation then a treatment then wants another within a few days do I have to go right through another form again?
All this is making me imagine a house full of forms if I am lucky enough to get a lot of business! I have a good filing system at the moment where I am keeping my forms Ive used up until now but surely I cant keep them forever?
I gave a lady an IHM the other day then she booked me for a reflexology treatment three days later, I took her IHM consultation form with me and just asked her if anything had changed since the last treatment rather than fill out a new one cos I really couldnt be bothered wasting time going right through all the questionaire again. ( I did record the foot reading though seeing as I was treating a different part of her body.)
I tried going through the forum to see if this has been covered and apologies if it has as I couldnt find it anywhere..
Id appreciate if others could tell me what they would do in this situation.

You could try having a ' follow up sheet'

A basic form that has clients details on - name, address etc but then have on it sections such as: any changes from previous/first consultation form, how client felt previous treatment went and any feedback. - this can help you save time from doing full consultation again and you gain feedback and can adapt/change/add to treatment if needed.


And your supposed to keep clients record for about 5 years.

Not sure if I'm right here but I are you not supposed to keep consultation forms for a certain amount of time for insuarance purposes? maybe someone else can answer this.
I use a simple follow up form which asks the client has there been any changes to their health or medication since their last treatment, providing it was relatively recent.

You have to keep consultation forms for 5 years.

Not just insurance, but HSE and regulatory bodies as well.

I use a full consultation forms for first time visits and a summary form for subsequent visits. My clients also sign to authorise the treatment.

Have you read your insurer's and Professional Association's record keeping requirements?

Compiling a 'follow up' sheet is a great idea.
I agree about the storage situation, heres hoping that we all make enough money to afford nice big offices to keep everything handy!

Insurance is 40 years guys (!!!), all others are up to 7 years. You aren't supposed to keep other peoples personal details for longer than this time (starting from when you stop treating them)

Insurance is 40 years? Keeping ex files? This is longer than most secret government/defence records.

I'm not sure what the ruling is for normal clients, but was told on a recent course that you must keep files on pregnant women for 25 years.

Can anyone advise whether I need to purchase consultation forms or are there some that aren't subject to copyright that I can use for Indian Head Massage. Any help would be appreciated.

I think it depends on your insurance company, might be best to ask them, I am sure the FHT said to keep them for 15 years !!



Why not try and make one yourself Maggie, I had to do this in my courses and used them til I bought mine from FHT

I ended up trawling through the internet for consultation forms i could use and found three that were openly available. I took what i liked from each of them and then created my own version. I now have a consultation form and a follow up form too which i am very happy with,,so far
