Intake Forms

I need to get intake forms. I think I have a book that has one that I can copy, but does anyone make their own intake forms, or do you have any websites or resources where I can buy some already made?
Also, does anyone add anything extra that they feel is necessary to their intake forms that are pre-made? Just curious.... :?:
I also find that forms from a Day Spa are a little different compared to say, a massage clinic.

The Business Mastery website has great forms that you can download and use. They have everything from SOAP charts and intake forms to HIPPA forms and release forms.

I took a basic form and made it my own. I added little diagrams so people can indicate where their problems are, and if there are any regions they don't want massaged. I also ask about job activity and hobbies because they can surely play a role in any issues the client is having. My form ended up being double sided, I don't think I'm asking too much information, and it doesn't take too long to fill out. Since I do outcalls, I hand it to the client while I am setting up for the first time. Works out quite well.

Thanks for the info everyone!
Oh, and another thing... at one of the spas I work at the Spa director was saying that even if a client signs a consent form it doesn't really protect you. How is that so? I mean, yeah if you injure your client then thats on you but if the client withholds some vital information about their health that was contraindicated to massage I think that is pretty important and could protect you, so why would she say that to us?
Another spa where I worked at had an intake form, but it was mainly just client information and massage experience and allergies and they did not sign for any agreement or consent.

I guess that it really depends on what is on the intake form. Mine ask about health history that pertains to the massage.

From our law & ethics classes, a consent form provides you some protection. It does not protect you from mal-practice. However too prove malpractice there must be negligance and harm caused to the client. As long as you stay within your scope of practice you'll be fine.
